Alleged or suspected food poisoning incident form

Alleged or suspected food poisoning incident form

If you are accused of causing food poisoning, you’ll need to gather information to defend your position. Use our form to do this effectively.

Helpful in formation

Do not admit liability at the time of the allegation. Be sympathetic to their symptoms and assure them a full investigation will be carried out, with the findings disclosed when completed.

An investigation should be carried out straightaway to either prevent any further illnesses occurring or to prove that your kitchen is not responsible.

Following an allegation any of the food product remaining should be labelled, isolated and stored in a refrigerator while investigations are carried out

Whilst you are encouraged to pass on details to a supplier where applicable, do not hand over personal details of the complainant unless the individual has given their consent.

All information should be retained for at least three years and kept in a secure location to ensure the safety of personal details.
